Getting the PT-141 dosage right matters for both effectiveness and tolerability. Too low and you may not see the desired effect. Too high and the side effects, particularly nausea, become more problematic. This guide covers the standard dosing framework, injection protocol, frequency considerations, and how to work with your physician to find the right dose for you.

The Standard FDA-Approved DoseThe FDA-approved dose of bremelanotide (PT-141) for women with hypoactive sexual desire disorder is 1.75mg, administered by subcutaneous injection approximately 45 minutes before anticipated sexual activity.
This is the dose used in the RECONNECT clinical trials that supported FDA approval, and it represents the established therapeutic standard for most adult women. The FDA label recommends against use more than once in 24 hours and suggests limiting use to a maximum of about 8 times per month.
For men using PT-141 (which is off-label use, as the FDA approval is specifically for premenopausal women), dosing is similar but physician-directed based on individual response.
How Dose Is Determined in PracticeThe FDA approval covers a single established dose level, but in clinical practice, some physicians use slightly different dosing based on patient response and tolerance. Compounded PT-141 allows for flexibility that the branded formulation doesn't, which means your physician can adjust your dose based on how you respond and whether side effects are manageable.
Starting at a lower dose (0.5mg to 1mg) and titrating based on response is an approach some physicians use for patients who are particularly sensitive to nausea or who have cardiovascular considerations that warrant more caution. If the lower dose produces the desired effect without problematic side effects, there's no reason to increase it.

Injection InstructionsPT-141 is administered by subcutaneous injection, meaning it goes into the fatty tissue just below the skin, not into muscle. The process is similar in technique to how insulin is injected.

Timing the InjectionThe 45-minute window before anticipated sexual activity is a guideline, not a precise threshold. Most women find the effects begin noticeably within 30 to 60 minutes and peak somewhere in the one to two hour range. Effects can last 12 hours or longer for some women, though the intensity of the initial response typically fades within a few hours.
Planning the injection around the 45-minute mark gives the medication time to absorb and reach peak plasma levels before the intended activity window.
Practical tip: nausea from PT-141 typically peaks within the first hour after injection. Planning injection timing so that the nausea window (if any) occurs before rather than during sexual activity is something many women find helpful. Injecting and then resting for 30 to 45 minutes before becoming active gives nausea time to pass for many people.
Managing Nausea at Different DosesNausea is dose-dependent with PT-141. At 1.75mg, nausea was reported in roughly 40% of women in clinical trials. At lower doses, the rate is lower. At higher doses, it increases.
For women who experience nausea at the prescribed dose:
Ask your physician about whether a dose adjustment is appropriate. Some women get adequate benefit at a lower dose with substantially less nausea, and your physician can assess whether that's the right move for you.
Take PT-141 on a fairly empty stomach. A heavy meal before injection tends to worsen GI symptoms.
Ginger in any form (tea, capsules, chews) has mild evidence as an anti-nausea remedy and is generally safe to use alongside PT-141.
Lying down for 30 to 60 minutes after injection lets the nausea window pass while you're at rest.
If nausea is severe or persistent beyond two hours, contact your EllieMD physician. Severe nausea may indicate that the dose needs adjustment.
Blood Pressure ConsiderationsPT-141 causes a transient blood pressure effect: an initial mild decrease followed by an increase. This typically resolves within 12 hours and doesn't affect most people meaningfully. But it's why the medication is contraindicated in women with cardiovascular disease or high blood pressure.
Your EllieMD physician will screen for these conditions before prescribing. If you begin taking a blood pressure medication after starting PT-141, notify your physician.
When PT-141 Is Not WorkingNot everyone responds to PT-141. About 60% of women in clinical trials reported improvement on key outcome measures, which means a meaningful subset didn't notice a clear benefit.
If you've used PT-141 at the prescribed dose three to four times without noticing any effect on desire or arousal, discuss this with your physician. A dose adjustment may help. Alternatively, other approaches, including hormonal interventions or therapy, might be more appropriate for your specific situation.
